Making an appointment:
Patients are encouraged to make prior appointment only. We are happy to see acute injuries following auto accidents or work injuries without an appointment. Our appointment desk is open from Monday through Friday from 8,30 a.m. until 5 p.m. Evening and weekend appointments are available on an as needed basis.
The initial visit
On your initial visit, it is recommended that you bring any X-rays, MRIs, CT Scans or any relevant test reports that have been performed with respect to your injury or painful spinal condition. Please be certain to bring your claim number and insurance information if you have sustained auto accident or work injury. We also need copies of your health insurance, pharmacy benefit card on day of your visit. Please make sure that you are expected to pay towards co-pay, deductibles and any uncovered charges at the time of your visit. It is also your responsibility to arrange for prior authorizations if required by your insurance. Please bring names, phone numbers and addresses of your primary care physicians, surgeons or lawyers involved in your medical care or injury treatment. Lack of authorizations and inability to prepay towards co-pay and deductibles might negatively affect our ability to provide services on the date of your consultation.
Prescriptions and renewals
As a policy, All medication prescriptions need office consultation. No medications are renewed by phone request .
Referrals
If you have been referred to us by your Primary Care Physician, please let us know his name, address and phone number so we can coordinate your care.
Accepted Insurance companies
All Auto Accident Insurance Plans, Worker's Compensation Insurance plans are accepted. Some service restrictions apply with Medicaid, Medicare, PPO and HMOs plans. Please contact our office to confirm your our ability to accept your insurance plans and any restrictions that might apply towards your care.
